Parents must make appointments
for enrollment/admission and encouraged to make an appointment upon
picking up the enrollment packets.
Enrollment Packets may be
picked up in the Counseling Office during regular school hours 7:00 AM
till 3:00 PM
This packet contains important information which will acquaint you with
Parkview, and the required forms for enrollment.
The forms that are printed on white or orange paper must be fully
completed. Please print this information in ink. This information will be
used to determine the student’s eligibility to enroll at Parkview and
proper grade level placement.
A postcard requesting your student’s records is also included. Please
complete both sides and mail this card to your student's former school. We
hope this will speed the transfer of records.
We ask that you allow one hour for the enrollment conference per
student to be enrolled. Without all proper documents, your student’s
enrollment can not be completed.
The information packet printed on blue paper is for you to keep. It
includes information on graduation, and college admission requirements,
immunization, International Newcomers Center and other items.
Please bring all of the following items listed below with you to the
enrollment conference. Missing information or documents will prevent the
student from enrolling. A more thorough explanation is included with the
white packet in this envelope. No exceptions can be made. (Counseling

Proof of Residence: Provide evidence demonstrating that you and your
child live in the Parkview attendance district (map on reverse) on a
full-time permanent basis. (Call 770-931-5611 for district and /or
transportation information.)

Proof of Parental Authority or Guardianship: The Affidavit of
Parental Authority (white packet) is required whenever a custodial or
guardianship change, divorce decree or separation agreement has occurred.
A Notary Public is available in the Counseling Office during the regular
school year, but not during the summer. Provide evidence demonstrating
that you are the guardian/custodial parent if there has been court
involvement.

Transcript, Withdrawal, Discipline Records, Social Security Number,
Immunization and Dental, Vision and Hearing Forms: Provide information
regarding the last school attended, including withdrawal form, academic
transcript, disciplinary record or letter of good standing on school
letterhead, and a copy of the student’s Social Security Card. We ask
that you provide both the I immunization and the Dental, Vision and
Hearing Forms at the enrollment conference. A postcard is enclosed for you
to obtain records from the previous school.
ENROLLMENT PROCEDURE DURING SCHOOL YEAR
Parent and Student:

Submit the packet, minus the information materials (blue paper) to
the counseling office.

The counseling secretary will schedule an enrollment conference.
Parkview Administration will:

Review all information to determine its completeness and accuracy
for enrollment, as well as determine if appropriate decisions and
placement for your child can be made.

If the information provided by the parent and student is not
adequate, an administrator (770-806-3794) will contact the
parent.

If adequate information is provided, the counseling secretary will
call to make an appointment for you and your child.
SUMMER ENROLLMENT ONLY
Parent and Student:

Counselors will:

Meet with you and your student in an enrollment conference.

Evaluate transcripts and select courses for fall semester

Give the entire enrollment packet to an administrator after the
conference for evaluation.
Administration will:

Review all information to determine completeness and accuracy for
enrollment, as well as determine if appropriate decisions and placement
for you child can be made.

If information provided is not adequate, Dr. Helen Thomas,
Administrator (770-806-3794) will contact the parent.

If all information is adequate, the student will be enrolled.
