PHYSICALS AND INSURANCE

  • Each player will be responsible for getting a physical from his or her own doctor. You will not be allowed to tryout unless this has been turned in.

  • All physicals expiring on or before April 1st must be renewed.

  • Parents must complete medical history and insurance waiver (on physical form).  Parents must sign in every place indicated on the physical form. 

ADDITIONAL INFORMATION

*Students will not be allowed to tryout unless the following things have been accomplished:

  • He/she passed at least five classes from first semester and is on track for graduation. This means that second year students must have 4 credits, third year students must have 10 credits, and fourth year students must have 16 credits. 

  • He/she has had a physical examination and turned in the completed form (including medical history and insurance form). If the student participated in another sport requiring a physical examination, he/she has one on file at Parkview.  He/she must indicate that to the coaches or see Mrs. Kelly in the main office to get a waiver form. Physicals are good for one calendar year and must not expire during the season.

  • He/she turned in the Drug and Alcohol form signed by both athlete and parents.

  • He/she turned in the Transportation Waiver form.

  • He/she turned in the Website Permission form.
